Are you coming to Northwest Veterinary Hospital for the first time? Or are you an existing client who has added a new four-legged friend to the family? Either way, we are glad to see you and your pets. We will do everything we can to care for and accommodate you and your dog or cat.

What to expect during your first visit:

You will want to arrive a few minutes early to get signed in and fill out the new client and/or new patient forms. You may print them out from the links below and fill them out ahead of time to make the process go even more smoothly. If you have any veterinary history from a previous clinic, please contact them in advance to have them fax medical and vaccine records to us at the fax number listed above. This allows our staff to familiarize ourselves with your pet before you even get here- as well as go through the paperwork to find out what vaccines or labwork your pet may be due for. This is necessary for a thorough examination. If you cannot get your old clinic to fax the records over, you may also bring hard copies of the paperwork to your first visit. However, we do prefer that we recieve it ahead of time.
